Hospital CEO pay in Massachusetts over four years: Whose pay increased the most?

Nonprofit hospitals are outliers in the nonprofit world when it comes to executive pay, paying their CEOs on average three times more than most other types of nonprofit organizations. On the highest end, we’ve seen the leaders of big hospital systems make a staggering $30 million+ in a single year.

How do the CEOs of hospital systems in Massachusetts stack up? We took a look at their compensation at large private nonprofit health systems (at least $1 million in total revenue) to find out.

How has hospital CEO pay changed?

Most CEOs saw an increase in pay from 2021-2024, averaging 29%, with the CEO of Mass General Brigham showing the most growth at 96%. Most CEO pay increases were greater than the rate of inflation (22%) and the increase in worker wages (26%) from 2020-2024.

However, there were some exceptions. At BMC Health System, CEO pay for Kathleen Walsh grew from about $2.3 million to $2.6 million (from 2021 – 2023), but when new CEO Alastair Bell took over, he was paid less than Walsh was in 2021. The bump in pay in 2023 reflects the pay for both CEOs who were active during the year. At Cape Cod Healthcare, CEO Michael Lauf’s pay was about $100,000 less in 2024 than 2021, although his pay did increase from 2021 to 2022 by about $1.4 million.

Not all of the CEO pay increases were linear; some saw downturns in pay in FYE 2023, which may have been due to financial challenges that many hospitals faced in that year. Most CEOs saw at least one 25% increase in a year, and four system CEOs had both a 25% increase and 25% decrease in different years over this time period. 

What’s behind CEO pay increases?

Unlike publicly traded for-profit companies, private nonprofits do not have to reveal the incentive structure for executives. This lack of transparency is ironic, since the tax exemption that nonprofits enjoy commonly assumes they are dedicated to a social mission missing at for-profit companies. Recent research provides some clues as to the potential drivers of CEO pay for nonprofit hospitals. Researchers at Rice University have found that from 2012-2019, hospital size, growth in that size, and growth in profits, were associated with CEO pay growth. Notably, financial assistance spending was not associated with increased CEO pay, while quality metrics were only weakly associated in 2019, especially when compared to earlier years.  

These patterns are somewhat reflected in the data for Massachusetts hospitals. Almost all of these hospital systems experienced revenue growth over the rate of inflation from 2021-2024, but did not increase their financial assistance spending as a share of expenses during this time.
